The information here is stale and needs to be updated from our other older sources, and then we need to contact them to verify what information we have. I'd prefer to have at the least addresses and phone numbers for each. I'd also like each to have their own page on which we can note a specific trans person's testimonial in dealing with that professional. I'd also prefer to have a list of qualifications each professional has (like Ph.D. and so forth).

If information does indeed get verified, place this template directly the heading for that particular doctor and and on his page, such as this example:
